Technical Assistance:
The following topics were discussed with the program director:
- documentation of driver qualifications
- diaper changing procedures/disposal of soiled diapers
- staff check of toys brought from home or another classroom

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector was in the facility between 11:00 AM and 2:00 PM. Ninety-eight children were in care upon arrival of the inspector. Morning activities/lessons, transition to lunch and lunch service were observed. The outdoor playgrounds were inspected. Medication administration and injury records were reviewed. A sample size of nine staff records and nine children's records were reviewed.

An exit meeting to review the Acknowledgement Form was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based upon review of nine staff records, the facility has not ensured that each staff record include documentation that staff have submitted results of a tuberculosis screening every two years.
Evidence:
The most recent documentation of a tuberculosis screening for staff 7 is dated 12/04/2017.

Plan of Correction: The facility responded with the following:
All staff records are being entered intro a program that tracks required documentation and sends alerts when updates are due.
Staff 7 will obtain a tuberculosis screening.

Description: Based upon observation, the facility has not ensured that hazardous substances are kept in locked locations using a safe locking method that prevents access by the children.
Evidence:
There was a container of shaving cream labeled "caution, keep out of reach of children" on a shelf above the counter in classroom 13.

Plan of Correction: The facility responded with the following:
Staff immediately removed the shaving cream to a locked storage cabinet. All staff will be reminded of potentially hazardous substances and the appropriate storage in locked cabinets.
