Comments:
The licensing inspector conducted an unannounced renewal inspection that began at the center at 9:45 am and concluded at the central office at 12:45 pm with staff record reviews. There were fourteen infants/toddlers in care with six staff. The children were observed playing with the available toys, playing with play dough and playing on the playground. Six staff's records and five children's records were reviewed. Violations from the previous inspection were also reviewed and two were repeated. Standard #: 22VAC40-185-140-A
Description: Based on record reviews, the center failed to obtain documentation of a physical exam.
Evidence:
1. The inspector reviewed five children's records. Two of five records reviewed (children's records #4 and #5) did not contain documentation of a physical exam. Both children have been enrolled for more than 30 days. Child #4's date of enrollment was 01/09/19 and child #5's date of enrollment was 11/15/19.

Plan of Correction: The family service worker contacted the children's parents during the inspection and requested a copy of their child's physical.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to have all hazardous substances stored in a locked place.
Evidence:
1. The inspector observed the cabinet under the changing table in classroom #1. There are two doors on the cabinet and one door was unlocked. The cabinet contained bleach water.

Plan of Correction: The teacher locked the cabinet during the inspection and will monitor to ensure it remains locked.
