Technical Assistance:
Today we discussed posting the inspection results so that each page may be read.

Comments:
An unannounced monitoring inspection was conducted today from 7:30 am through 8:30 am. There were 20 children in care five through ten years old with two staff. Children records and staff emergency contact information were reviewed on site. Staff records are housed at a central location and will be reviewed at a later date. A walking of the center inside was completed. Required postings, annual inspections, emergency evacuation drills first aid kit, medication and transportation were reviewed. Children were observed in teacher and self-directed activities including restroom routines and learning center play. Violations regarding supervision emergency radio and long-term medication authorization were identified and appear on the violation notice. The violations were discussed with the site director at the conclusion of the inspection at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on observation the center failed to ensure that children under 10 years of age always are within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom.
Evidence:
1 - Child 1, who stated she was six years old, left the classroom alone. A short time later child 1 returned to the room and said "can someone come open the bathroom door for me." She stated this twice. A child sitting at the snack table got up ran out of the room and returned a short time later.
After approximately six minutes child 1 returned to the classroom.
a. The bathroom is located approximately 60 feet from the child care room and through two sets of double doors, a third door and around a corner.
2 - When asked the name of the child who left the room to assist child 1, staff 1 was not aware that the two children had left the room.

Plan of Correction: The bathroom policy was reviewed with the entire staffing team. Emphasis was placed on all children being in sight and sound at all times and students are always to be escorted by staff in a grouping of no less than 3 to use the restroom.

Standard #: 22VAC40-185-510-C
Description: Based on record review the center failed to ensure that long-term prescription drug use and over-the-counter medication may be allowed with written authorization from the child's physician and parent.
Evidence:
1 - There was an Epi Pen Jr. in the center medication lock box that does not have written authorization from the child's physician.
a. The parent authorization signature was dated November 20. 2019.

Plan of Correction: The center will notify the parent to obtain the required physician?s signature on the YMCA Medication Administration Form to ensure long- term prescription drug use and over-the-counter medication for the student.

Standard #: 22VAC40-185-540-E
Description: Based on observation and interview the center failed to ensure that was one working, battery-operated radio in each building.
Evidence:
Staff 1 stated that there were not any batteries for the emergency radio.

Plan of Correction: The center will purchase a battery operated radio along with extra batteries for backup.
