Comments:
An unannounced monitoring inspection was conducted on Thursday, February 27, 2020 to determine the center's compliance with licensing standards. The inspector arrived at 9:30 AM and concluded the inspection at 11:45 AM. The census for today's inspection consisted of 22 children. During the inspection, children were writing letters of the alphabet, having storytime, playing a counting game, painting, bathroom breaks, washing hands, eating lunch and napping. Children's equipment, learning materials, indoor and outdoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed. The provider stated that medication is not administered at this time. Five children's records and five staff records, annual fire and health inspections, fire drill, lockdown drill and shelter in place drill logs, daily attendance and all required postings were reviewed.

The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence:
1. The record for child #4 (date of enrollment 11/18/2019) had a physical examination dated 01/02/2019.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, the center did not ensure that one of five records reviewed contained documentation that two or more references as to character and reputation as well as competency were checked before employment.

Evidence:
The record for staff #2 (date of hire 05/28/2019) had two reference checks completed on 06/04/2019.

Plan of Correction: Policy/procedure will be reinforced on the time that documents have to be in before hiring.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence:
1. In room #5 there was a one inch by one inch hole in the wall and an area of baseboard was peeling away from the wall, exposing drywall.
2. Children ranging in age from 1 year old to 5 years old use room #5 to eat and nap.

Plan of Correction: The hole was patched and the baseboard was fixed during the inspection.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the facility did not ensure diapering standards were followed.

Evidence:
1. In bathroom #3, the plastic-lined storage system had a broken operational foot operated pedal. The standard requires that the plastic lined storage system is either foot-operated or used in a way that neither the staff member's hand nor the soiled diaper touches the exterior surface of the storage system during disposal.
2. The changing table and supplies are located in bathroom #3.

Plan of Correction: Replace the trash can today.
