Comments:
An unannounced monitoring inspection was conducted on Monday, March 9, 2020 to determine the center?s compliance with licensing standards. The inspector arrived at 9:50am and concluded the inspection at approximately 12:20pm. The center?s directors and owners were present and assisted the inspector throughout the inspection. The census for today?s inspection consisted of 43 children in the direct care of 12 staff members. Upon arrival, the children were engaged in a variety of activities. Each classroom was inspected and formal observations were completed. The center is equipped with age and stage appropriate materials and equipment for the children?s use. All areas where the children receive care, inside and outside, were inspected. The center had the following posted: license, daily schedules, emergency numbers, evacuation maps, menu, special orders, and various parent information. The center?s first aid kit and non-emergency supplies were inspected and found complete. During the inspection, five children?s records and five staff records were reviewed. Medication is administered and three emergency medications were reviewed today. The most recent evacuation drill was documented on 03/04/2020. The most recent shelter-in-place drill was documented on 09/17/2019.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation on 03/09/2020, the center did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.

Evidence: 1) During the inspection of the school age playground, the licensing inspector observed one of the railroad ties around the playground border was dry rotted. The dry rotted railroad tie had an upright protruding nail completely exposed. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Plan of Correction: The railroad tie will be replaced and the protruding nail covered until the repairs are complete.
