Comments:
An unannounced monitoring inspection was completed on 03/04/2020 from 10:00 A.M.-1:00 P.M. At the time of the inspection 46 children were in care of seven staff and one administrator present. Five children records, five staff records, two medications and required documentation,over-the-counter topical skin ointments and required documentation, six injury reports, first aid kits, emergency supplies, evacuation drills, health inspection, fire inspection, playground, and posted required information were reviewed. Children were observed during morning outdoor play time, diapering, handwashing, morning snack, behavioral guidance, circle time, transition to outside, weeding and preparing gardens for planting, lunchtime, transition to naptime, and departure of half day children. The exceptions to compliance are noted on the violation notice with two repeat violations cited.

Violations:
Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to ensure that all electrical outlets were covered with a protective cover.

Evidence:
1. In Classroom #1 there was one electrical outlet near the sink and counter that did not have a protective cover.
2. In Classroom #2 there was one electrical outlet behind the movable metal shelve that did not have a protective cover.
3. Staff verified that the electrical outlets did not have protective covers.

Plan of Correction: Staff immediately located protective covers and placed in the electrical outlets. Administration will review with all staff the requirement regarding electrical outlets.

Standard #: 22VAC40-185-520-C
Description: Based on observation, the center failed to ensure that all diaper creams were inaccessible to children.

Evidence:
1. In Classroom #1, there was a Frozen backpack in a cubbie, that had a bottle of Aquapor diaper ointment in the bag. The backpack was accessible to children.
2. Staff verified that the diaper cream was located in the backpack, and that the backpack was accessible to children.

Plan of Correction: Staff was unaware that there was diaper ointment in the back pack. Staff will notify parents to give over-the-counter-ointments to staff so that they can be stored properly. Staff immediately removed the diaper ointment from the bag and placed in a locked cabinet.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the center failed to ensure that all food brought in from home was dated.

Evidence:
1. In Classroom #1 lunchboxes that were stored in the children's cubbies were not labeled with the date.
2. Staff verified that the lunchboxes were not labeled with the date.

Plan of Correction: Administration will discuss with the staff the requirement for all food brought in from home to be dated. The administration will work with the teachers to figure out a solution that will meet the requirement.
