Comments:
An unannounced monitoring inspection was completed on 03/10/2020 from 10:10 A.M.-2:00 P.M. At the time of the inspection 87 children were in care with nine staff and two administrator present. Eight children records, eight staff records, 5 medications and required documentation, five over the counter topical ointments and documentation, three injury reports, first aid kits, emergency supplies, playground, health inspection, fire inspection, playground safety plan, and posted required information were reviewed. Children were observed during morning work cycles that included math activities, handwriting, literacy, fine motor activities, quiet reading time, AM snack time, outside play time, handwashing, lunchtime, 1/2 day departure, naptime, and afternoon work cycles. The exceptions to compliance are noted on the violation notice.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of staff records, the center failed to obtain the results of a follow-up tuberculosis screening every two years after the first initial screening or testing.

Evidence:
1. The record for Staff #4 did not contain documentation of an updated TB test or screening. Last documented update was 01/09/2018.
2. Staff verified that the record did not contain documentation of an updated TB test or screening.

Plan of Correction: Administration will contact Staff #4 and request an updated tuberculosis screening. The employee will not work at the center until the tuberculosis screening is obtained.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center failed to ensure that children washed their hands with soap and running water, or a disposable wipe after eating snack and meals.

Evidence:
1. In Classroom #1 children were observed finishing snack. The children put their dishes in the sink and threw away their trash. The children were observed then going to circle time. The children did not wash their hands with soap and running water or a disposable wipe after snack time.
2. In Classroom #2 children were observed finishing and cleaning up lunch. The children cleaned up and went to join story time. Children did not wash their hands with soap and running water or a disposable wipe after lunch time.

Plan of Correction: Administration will speak with all staff to review all requirements for handwashing.

Standard #: 22VAC40-185-520-D
Description: Based on review of insect repellents, the center failed to ensure that all containers were labeled with the child's name.

Evidence:
1. There was a container of Off Insect Repellent in the storage container outside on the preschool/kindergarten playground that was not labeled with the child's name.
2. There was a container of Repel Natural Insect Repellent in the storage container outside on the preschool/kindergarten playground that was not labeled with the child's name.

Plan of Correction: Administration will meet with all staff and review the requirements for insect repellent and all over-the-counter skin products. Administration will do spot checks to make sure all over-the-skin-products and insect repellents are labeled with the child's name.
