Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on February 7, 2020 regarding allegations of inadequate supervision, food and nutrition, physical plant and ratios. The inspection was initiated on March 10, 2020 and completed on March 10, 2020. On March 10, 2020, the inspector was on site from 11:10am to 12:35pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.

The preponderance of evidence gathered during the investigation supports part of the allegations; therefore, the complaint related to ratio is determined to be valid, but the complaint related to supervision, food and nutrition and physical plant is determined to be not valid. Description: Based on documentation, observation and a staff interview, ratio requirements of one staff member for every five children was not met for children 16 months old to two years in the Toddler classroom.
Evidence:
1. Upon arrival, the inspector observed one staff member supervising six children in the Toddler classroom, exceeding the ratio by one child.
2. According to the center's sign in sheet, the sixth child arrived at approximately 8:35am. An additional staff member entered the room at approximately 11:20am.
3. A staff interview confirmed that the Toddler classroom did not meet the required one staff member for every five toddler children on the day of the inspection from about 8:35am to 11:20am.

Plan of Correction: One of the students was temporarily moved to another class until the return of the co-teacher as [they] were out due to sudden hospitalization for an illness. Responsible Person: [Director]
