Comments:
An unannounced monitoring inspection was conducted today from 9:30 am - 12 noon. There were 21 children in care ages 12 months through five years old with four teachers. Children and staff records were reviewed. Required postings, annual inspections, emergency evacuation drills, first aid kit and medication were reviewed. A walking of the center inside and outside was completed. Children were observed engaged in teacher and self-directed activities including restroom routines, transitions hand washing, lunch, and inside play activities. Violations regarding tuberculosis screening, physical plant, emergency supplies and transportation were identified and appear on the violation notice. The violations were discussed with the owner at the conclusion of the inspection.

Standard #: 22VAC40-185-160-C
Description: Based on record review the center failed to ensure that at least every two years from the date of the first initial screening or testing staff members obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
1 - The most recent tuberculosis screening on file for staff 2 was dated May 27, 2017 and therefore has expired.
2 - Staff 1 confirmed that the tuberculosis screening on file for staff 2 has expired.

Standard #: 22VAC40-185-270-A
Description: Based on observation and interview the center failed to ensure that areas and equipment are maintained in operable condition.
Evidence:
1 - On the play ground there was a yellow, toy shopping cart that had areas cracked plastic within reach of children.
2 - There is a plastic base to a large shade umbrella that has come apart and poses a tripping hazard.
3 - Staff 1 confirmed that there is a toy shopping cart with cracked plastic and an umbrella base that is coming apart on the playground.

Description: Based on observation and interview the center failed to ensure that there is a one-day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care maintained at the center.
Evidence:
1 - There was not a one-day's emergency supply of disposable bottles, nipples, and commercial formulas available for review during the inspection.
2 - Staff 3 stated that the center did not have a one-day's emergency supply of disposable bottles, nipples, and commercial formulas for all infants present for care.
