Technical Assistance:
We discussed the requirements for diapering procedures. The risk assessments for violations were provided.

Also, we talked about your written diapering procedures posted for staff.

Comments:
A complaint received on February 24, 2020 was investigated during unannounced inspections on March 4, 2020 from 9:53 AM to 1:45 PM, and on March 6, 2020 from 9:48 AM to 11:35 AM, regarding diapering procedures, supervision, staffing and the care and protection of children. After completing interviews by phone on February 24, 2020 and in person at the center on March 4, 2020 and March 6, 2020, observing the care of children, and reviewing written procedures, it was determined that the complaint concerning diapering procedures was valid. There were not valid findings regarding supervision, staffing, and the care and protection of children. A violation notice and plans for correction were provided. Description: Based on interviews with staff, and observations of the care of children, the center failed to ensure that staff have always washed their hands with soap and running water before and after a diaper change. Evidence: During staff interviews on March 4 and 6, 2020, it was confirmed that as many as five different staff who have worked with the early one's group did not always wash hands after and before diaper changes when multiple children needed diaper changes and there were children freely moving around the classroom for activity transitions. The staff indicated that some of these times either they or other staff who conducted diaper changes used hand sanitizer instead of soap and running water at a sink.

Plan of Correction: The director immediately began addressing the requirement for hand washing with soap and water after each diaper change and before changing a diaper The instructions posted in the room at the changing tables with the title "Recommended Procedure For Diapering A Child" were replaced with instructions that state they are required procedures. The director or assigned teacher will conduct periodic observations to ensure that all staff follow the procedures for hand washing at sinks. As needed, staff will be retrained in diapering procedures.

Description: Based on interviews with staff, and observations of the care of children, the center failed to ensure that all staff implement required diaper changing procedures. Evidence: Staff interviews confirmed that multiple staff who have changed diapers in the older infant/younger toddler room have not consistently cleaned the diapering surface with soap and at least room temperature water and sanitized the mat after each use. Staff indicated that when there were multiple children needing diaper changes and a number of children involved in free choice of activities in the room, staff have skipped the cleaning and sanitizing process between diaper changes in order to complete the routine sooner.
In addition, the diapering surface has not always been used only for diapering or cleaning children. On the morning of March 6, 2020, the licensing inspector observed a storage bin, clipboard and craft paper placed temporarily, at different times, on the diaper changing mat in the older infant/younger toddler room.

Plan of Correction: The director immediately began addressing the requirement for both cleaning and sanitizing the diapering surface after each diaper change with each staff member. The instructions posted in the room at the changing tables with the title "Recommended Procedure For Diapering A Child" were replaced with instructions that state they are required procedures. The director or assigned teacher will conduct periodic observations to ensure that all staff follow the procedures for using the cleaning solution and the sanitizing solution on the diapering surface. As needed, staff will be retrained in diapering procedures. Also, the staff were instructed to use the diapering surface for diaper changes and not as a place to put things on.
