Description: Based on record review, children's records were not complete with all required information.

Evidence:
3 of 9 records reviewed were missing required information:
1. Child A was missing father's work address.
2. Child C was missing the work address for the mother and the father.
3. Child I was missing the father's work phone number.

Plan of Correction: Information will be completed at time of enrollment. Information was present in file and printed out email chain.

Description: Based on review, staff records did not have all the required information.

Evidence:
1. Staff B was missing proof of age.
2. Staff C was missing orientation.
3. Staff D was missing proof of age.
4. Staff E was missing job title and proof of age.
5. Staff F was missing proof of age.

Plan of Correction: Some were corrected during inspection. Director stated staff will complete the required documents.

Description: Based on record review, all staff did not complete at least 16 hours of training.

Evidence:
Staff F completed 7 hours of training in 2019.

Plan of Correction: Director stated staff left training in her backpack located in her home. CDA Certificate was in file completed in 2019 which is 120 hours of training.

Description: Based on observation, hazardous substances were not kept in a locked pace.

Evidence:
1. Disinfectant spray was found in an unlocked cabinet located in the "Early Preschool" and "Two's 2" rooms

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-350-E-4
Description: Based on record review, the ratio for three year old children was not maintained.

Evidence:
1. The "Name to Face" record taken on 03/02/2020 for the "Preschool" room showed staff were out of ratio from 8:19 am until 9:00 am. The record shows at 9:00 am there were 20 three year old children in the care of one staff (Staff G).
2. The "Name to Face" record taken on 03/02/2020 for the "Preschool 2" room showed from 9am-9:37am there were 13 three year old children in the care of one staff (Staff G). The record shows during the 9:00 am hour there was 11 three and four year olds to 1 staff and during the 10 o'clock hour there were 13 three and four year olds to one staff.
3. Staff G confirmed she was alone from 9am-9:37am with 13 three year old children.

Plan of Correction: Staff Schedules have been changed to accommodate the peak drop off times in the morning.

Standard #: 22VAC40-185-420-E-3
Description: Based on record review, Staff did not provide at least semiannual scheduled opportunities for parents to provide feedback on their children and the center's program.

Evidence:
A. Child E's record was last reviewed 08/18/2018.
B. Child F's record was not reviewed in 2019.

Plan of Correction: System set up to review parent files semiannually.

Standard #: 22VAC40-185-550-E
Description: Based on record review and interview, emergency evacuation drills were not recorded.

Evidence:
Staff A stated an evacuation drill was conducted in September 2019 and not recorded.

Plan of Correction: Fire drill was recorded from email print out in binder. Fire drills will be recorded on same form.
