Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on May 8, 2020 and concluded on May 11, 2020. The director was contacted by telephone to initiate the inspection. There were seven children present and five staff. The inspector emailed the director a list if items required to complete the inspection. The inspector reviewed two children's records, and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of children records, the center failed to obtain a completed physical examination for one child before the first day of attendance or within one month after attendance.

Evidence:
1. Child #2's paperwork did not contain a completed physical examination. Date of enrollment for Child #2 was 01/02/2020.
2. Staff verified that there was not a completed physical examination in the child's file.

Plan of Correction: Staff will contact the parent's of Child #2 and request a completed physical examination. In the future the center will track new enrollments to the center and the time frame of when physicals are due.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children records, the center failed to ensure that all required information was obtained and kept on file.

Evidence:
1. Child #1's registration form was missing the place of employment and phone number of employment for both parents. Date of enrollment for Child #1 was 09/25/2017.
2. Child #2's file was missing a complete emergency contact address for one of the emergency contact listed. Date of enrollment for Child #2 was 01/02/2020.

Plan of Correction: Administration will contact both sets of parents to obtain required information. In the future all registration forms will be reviewed upon enrollment to ensure all required information is obtained.

Standard #: 22VAC40-185-90--A
Description: Based on review of children records, the center failed to ensure that a written agreement between the parent and the center shall be in each child's record by the first day of the child's agreement.

Evidence:
1. Child #1's file did not contain a written agreement for the following agreements. A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center, and a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease.
2. Staff verified that there was not a written agreement between the center and the a parent that included statements regarding pick up of a child when they become ill, and notification to the center within 24 hours or the next business day after the child or any member of the immediate household has developed a reportable communicable disease.

Plan of Correction: New administration started at the center in October 2019. They have been reviewing agreements and updating forms. The administration was aware that the parents had not signed the updated agreements. Parents will be contacted and asked to complete the updated agreement form.
