Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 05/19/2020 and concluded on the same date. The facility representative contacted by telephone to initiate the inspection. There were no children present and no staff. The inspector emailed the facility representative a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, one of two records reviewed did not contain the results of a follow-up tuberculosis (Tb) screening at least every two years from the date of the first initial screening or testing.
Evidence:
1) The record for Staff #2 did not contain a current Tb screening result. The most recent screening result available for review was dated 09/12/2017.

Plan of Correction: Staff will have the screening done once they can coordinate an appointment with their doctor's office. (Due to COVID-19)

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review and interview, one out of two records reviewed did not contain documentation of the result of an out-of-state Central Registry search request within 30 days of employment for employees that had lived out of the state of VA within the last 5 years.
Evidence:
1) The record for Staff #2 (start date 08/28/2017) did not contain documentation of an out-of-state Central Registry search results. Staff #2 had declared in their sworn statement having lived out of the state of VA within the last 5 years before employment.
2) The facility representative located in Staff #2's record documentation that the request was submitted but no results were located in the record.

Plan of Correction: We will investigate with the out-of-state office to check status on this background check.
