Comments:

A Subsidy Health and Safety inspection was conducted on 03/09/2020 from 9:20 am to 11:30 am to review supplemental health and safety requirements. A formal observation was completed in the Toddler, Three-Fives?, and Head Start classrooms. The children were observed singing, playing with puzzles, identifying shapes and letters, and reading books. The children in the Head Start program were observed participating in a lesson about authors, illustrators, and parts of a book. Lunch consisted of beef tacos with shredded cheese, lettuce, tomato, fruit and milk. The lunch menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All areas utilized by children were inspected. Records were reviewed for five children. The staff records are kept off site and will be checked at a later date. The staff health and emergency contact information was reviewed. Medication is being administered and was reviewed. The center?s injury prevention plan was updated on 12/20/2019. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 02/18/2020 Last shelter-in-place drill: 12/20/2019 Last fire inspection: 09/26/2019 Last health inspection: 11/21/2019 Census: There were 20 children and five classroom staff in attendance during the inspection. There were no repeat violations found during the inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description:	Based on record review and interview, the vendor failed to ensure that an annual lockdown drill was conducted. Evidence: 1. The last recorded lockdown drill was dated 10/11/2018 2. The director confirmed that a lockdown drill was not conducted in 2019.
Plan of Correction:	I will complete a lockdown drill on 03/18/2020 with a normal file drill.
