Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on May 28, 2020 and concluded on May 29, 2020. The director was contacted by telephone to initiate the inspection. There were 28 children present and eight staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed three children?s records and three staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of three children's records and interview, the center did not obtain documentation of immunizations for one child prior to the child attending the center.
Evidence:
1. The record for Child #1 (start date 01/27/2020) contained documentation of immunizations dated 03/04/2020.
2. Staff #1 confirmed this information during the remote inspection.

Plan of Correction: Management will confirm the parent provides documentation of immunizations before their first day of care. Newly enrolled children will not be allowed to attend the center until immunizations have been received. The center is currently under new management.

Standard #: 22VAC40-185-140-A
Description: Based on review of three children's records and interview, the center did not ensure two children had a physical examination by or under the direction of a physician prior to or within one month after attending the center.

Evidence:
1. The records for Child #1 (start date 01/27/2020) and Child #2 (start date 07/10/2019) did not contain documentation of a physical exam.
2. Staff #1 stated both families had been notified and a physical exam had been requested.

Plan of Correction: Documentation of physical exams were requested by management on the day of the inspection. Management will obtain the physical examination within the required time frame for newly enrolled children. The center is currently under new management.
