Standard #: 22VAC40-185-60-A Description: Based on a review of three children's records, the center did not ensure to that one child's record contained the required information.
Evidence: 1) The two emergency contacts in the record for Child #1, enrolled on 02/25/2019, did not contain a complete address. Each child record shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.Plan of Correction: Not available online. Standard #: 22VAC40-185-70-A Description: Based on a review of three staff records, the center did not ensure two staff records contained the required information.
Evidence: 1) One of the two phone references in the record for Staff #1, hired on 02/26/2020, did not contain the signature of the person making the call. 2) One of the two phone references in the record for Staff #2, hired on 11/04/2019, did not contain documentation of the dates of contact and the signature of the person making the call. Each staff record should contain documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering. If a reference check is taken over the phone, documentation shall include: dates of contact; names of persons contacted; the firms contacted; results; and signature of person making the call.Plan of Correction: Not available online.
