Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on May 6, 2020 and concluded on May 7, 2020. The director was contacted by telephone to initiate the inspection. There were no children present and 1 staff. The Inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of records on May 7, 2020, the center did not ensure each child's record contained all the required information.
Evidence: The record of Child #1 (DOE 9/16/19) and Child #2 (DOE 9/16/19) did not contain documentation of viewing proof of the child's identity and age.

Plan of Correction: Director of Education and Site Coordinators will review process for enrolling students ensuring all forms are filled accurately and completely.
