Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on June 16, 2020 and concluded on June 16, 2020. The person in-charge was contacted by telephone to initiate the inspection. There were no children present and no staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on a review of documentation and a staff interview, the center did not provide documentation of an annual fire inspection. Evidence: 1. The licensing inspector observed the most recent fire inspection available to review during the inspection was dated 01/28/2019. 2. Staff reported a more recent fire inspection had been completed, but could not locate documentation.

Plan of Correction: Administration will provide the most recent fire inspection to the licensing inspector.

Standard #: 63.2(17)-1721.1-A
Description: Based on a review of three agent records, two records did not contain documentation of background checks within the required time frame. Evidence: 1. The licensing inspector observed the record for Agent #1 contained a Central Registry finding dated 8/8/14. 2. The record for Agent #2 contained a Central Registry finding dated 8/8/14. Central Registry findings are required to be updated every five years.

Plan of Correction: Administration will ensure that the agents submit the Central Registry search requests no later than 6/26/2020. Results will be provided to the licensing inspector.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on review of three agent records, the center did not obtain a national fingerprint-based criminal history record check for one listed agent. Evidence: The record for Agent #3 did not contain documentation of a national fingerprint-based criminal history record check. A statement of eligibility based on a national fingerprint-based criminal history record check was required to be obtained prior to 9/30/2018.

Plan of Correction: Administration will ensure that documentation of the agent fingerprinting is provided to the licensing inspector. If the results cannot be located, the agent will make an appointment no later than 6/26/2020.
