Technical Assistance:
Provided consultation on procedures for cleaning and sanitizing -
1) "Cleaned" means treated in such a way as to remove dirt and debris by scrubbing and washing with soap and water or detergent solution and rinsing with water; "Sanitized" means treated in such a way as to remove bacteria and viruses from inanimate surfaces through first cleaning and secondly using a solution of bleach water prepared fresh daily. The surface of the item is sprayed or dipped in the sanitizing solution and then allowed to air dry.
DSS has model forms for long term medication administration that can be found on the dss website at dss.virginia.gov.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 7/28/2020 and concluded on 7/29/2020. The director was contacted by telephone to initiate the inspection. There were 30 children present with five staff supervising. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed three children's records and three staff records to ensure documentation was complete. The information gathered during the inspection determined non-compliance with standards that are documented on the violation notice issued to the facility.

The LI also reviewed documentation regarding adding an additional licensed room/space. Based on the measurements received, the space can accommodate 22 children.

Violations:
Standard #: 22VAC40-185-510-C
Description: Based on review of four medications and authorization forms, it was determined that not all required procedures for administering long term medication were followed. Evidence: LT medication #1 for child A did not have a parent's signed authorization; LT medication #2 for child A did not have a physician's signed authorization; and LT medication #3 for child A did not have a parent or physician's signed authorization form.

Plan of Correction: Will obtain the missing signatures on the medication authorization forms.

Standard #: 22VAC40-185-560-K
Description: Based on an interview with staff, the facility failed to clean the tables as required. Evidence: staff stated that they do not use soap and water to clean the tables before they are sanitized.

Plan of Correction: Will begin using soap and water to clean tables.
