This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection as initiated and concluded on August 4th, 2020. The director was contacted by telephone t initiate the inspection. There were 55 children present and 11 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children's records and 4 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-70-A Description: Based on a review of staff records, there was no documentation that staff persons received orientation training.
Evidence:
At the time of the inspection, 4 of 4 records did not contain completed documentation and signature that staff had completed orientation training.Plan of Correction: We have developed a new orientation form that will be signed by person delivering orientation and the date of delivery and the signature of the person receiving orientation and the date.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of staff records, the licensed welfare agency does not have a central registry finding within 30 days of employment .
Evidence:
Staff #4 does not have a central registry finding within 30 days of employment. Date of employment 01/13/2020.Plan of Correction: I will contact OBI (forms were mailed 1/17/2020 and 2/18/2020) to research completed form.
