Standard #: 22VAC40-185-340-G Description: Based on staff interview and a review of pictures, the center did not have a system to ensure that staff know where the children are and what they are doing.
Evidence:
1. At approximately 1:30 pm on 07/14/2020, child 1, age ten years, attempted to climb a tree without knowledge of staff 1. During this attempt, child 1 fell resulting in a compound fracture to her right forearm.
2. Staff 2 stated she told the children to line up while she put away the art supplies and washed her hands.
a. Staff 2 stated, " I heard crying and looked up and saw child 1 on the ground." "I walked over to inquire as to what had happened and was told child 1 had tripped on her shoe lace while trying to climb the tree."
b. Staff 2 additionally stated she was not aware that child 1 had attempted to climb the tree. " I was bending down to wash my hands when I heard crying. The children told me that child 1 had tried to climb the tree and tripped on her shoe lace."Plan of Correction: The Hilltop Family YMCA Childcare has reviewed and updated our policies and procedures on playground safety and supervision of children. We conducted a staff meeting on August 12, 2020 to review playground safety and child supervision. There will be a minimum of two staff on the field with a group outside at any given time. Staff shall follow the playground safety best practices. Staff will ensure proper name to face head counts and keep children within sight and sound supervision.
Standard #: 22VAC40-185-340-H Description: Based on interview, the center failed to ensure that when the outdoor activity area is not adjacent to the center, there shall be at least two staff members on the outdoor activity area whenever one or more children are present.
Evidence:
1. There was one staff present with 7 school age children at approximately 1:30 pm on 07/14/2020 according to information provided by staff 1 and staff 2.
a. Two staff are required because the outdoor play area is not located next to the facility.
b. Children play in a field across the parking lot from the facility.
2. Staff 1, who is the center director, stated it was the usual practice of the facility, to have two staff on the outdoor play area at all times.Plan of Correction: The Hilltop Family YMCA Childcare has reviewed and updated our policies and procedures on playground safety and supervision of children.
