Comments:
A subsidy health and safety inspection (SHSI) was conducted on May 16, 2022. There were two children present with one caregiver supervising. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-665-230-H
Description: Based on interview and review, vendor did not ensure caregivers complete 16 hours of annual training to include the department?s health and safety update course.

Evidence: Vendor had documentation of completing 3 hours of training in 2021. Vendor stated she has not completed the 3 hour department health and safety update course.

Plan of Correction: Vendor will complete the course online by 5/23/2022 and ensure 16 hours of training are completed in 2022.

Standard #: 22VAC40-665-250-A
Description: Based on observation, vendor did not ensure potentially poisonous substances are stored away from food in areas inaccessible to children.
Evidence: Liquid bleach, bleach tablets and laundry detergents were stored on a floor rack by the washing/drying machines within reach of children in care. Bug spray was stored on the floor by the front door within reach of children in care. These items were labeled ?caution or warning? and ?keep out of reach of children?.

Plan of Correction: Bleach and laundry detergents will be kept on top of the stackable washer and dryer that is out of reach of children. The bug spray will be kept out of reach of children as well.

Standard #: 22VAC40-665-370-E
Description: Based on observation and interview, vendor did not ensure the diapering surface is nonabsorbent.

Evidence: Vendor stated she changes children?s diapers by laying the child on a disposable pad on her bed. Vendor demonstrated by laying a disposable pad directly on the bed comforter.

Plan of Correction: Vendor will use a nonabsorbent diaper pad under children when she changes their diapers.
