Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 8/28/2020 and concluded on 8/28/2020. The director was contacted by telephone to initiate the inspection. There were 76 children present and 15 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of four children's files, the facility failed to have all of the required information in each child's file. Evidence: Child 1 did not have documentation of the address for one emergency contact.

Plan of Correction: Director will request the emergency contact address from the parent.

Standard #: 22VAC40-185-90--A
Description: Based on review of four children's files, the facility failed to have documentation of a written agreement between the parent and the center by the first day of the child's attendance. The written agreement includes the authorization for emergency medical care, the parents agreement to pick up the child when ill, and that a communicable disease will be reported within 24 hours. Evidence: Child 4 (date of enrollment 4/13/2020) did not have documentation of the written agreement on file.

Plan of Correction: If the signed form can not be located, the director will get a new form signed by the parents.
