This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 08/26/2020 and concluded on 08/26/2020. The in-charge person was contacted by telephone to initiate the inspection. There were 77 children present and 12 staff. The inspector emailed the in charge person a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.
The information gathered during the inspection determined violations with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-130-A Description: Based on record review the licensee did not ensure , each child has recieved the immunizations required by the State Board of Health before child attends the center.
Evidence:
Child #2 start date 5/20/2020 did not obtain immunization document for child #2 prior to starting. Staff #1 confirmed document was not received from parent before start date.Plan of Correction: Center stated they have contacted the parent for the documentation.
Standard #: 22VAC40-185-140-A Description: Based on record review and staff interview, the licensee did not ensure the physical was obtained before the child attends or with in one month after attendance.
Evidence: Child #3 start date 08/04/2015 did not have a physical documentation available in his record. Staff #1 confirmed he did not see the physical in the record.Plan of Correction: Director located the physical.
Standard #: 22VAC40-185-60-A Description: Base on record review and staff interview, the licensee did not obtain all required documents for each enrolled child.
Evidence: Child #2 start date 05/10/2020 did not have documentation for proof of identity. Staff #1 confirmed he does not have documentation of proof identity but has contacted parents.Plan of Correction: Director received both documents form the parent.
