Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An initial inspection was initiated on September 4, 2020 and concluded on September 4, 2020. The director was contacted by telephone to initiate the inspection. There were 0 children present, 2 board members/prospected owners and 1 volunteer. The Inspector conducted a virtual inspection to complete the inspection. The Inspector reviewed 0 children?s records and 2 board members/prospected owners record submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation on September 4, 2020, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: The 4 year old classroom contained a bench with foam seat. The seat was ripped on two corners exposing the inner foam, which is a choking hazard. The rips were approximately 1-2 inches around the two front edges of the foam bench seat.

Plan of Correction: The seat cushion has been removed from the classroom and the rip repaired. The owner and directors will review cushions frequently to make sure all are in compliance. Persons responsible: Owner, Director 1 and Director 2.
