Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 9/16/20 and concluded on 9/17/20. The director was contacted by telephone to initiate the inspection. There were 51 children present and 7 staff present. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-550-D
Description: Based on a review of the emergency drill log and interview, it was determined that the facility did not ensure that a monthly practice evacuation drill is completed.

Evidence:
1. The emergency drill log did not have written documentation to demonstrate that an emergency evacuation drill was completed during the month of August 2020.
2. Staff #4 (Program Director) confirmed that there was no emergency evacuation drill completed during the month of August 2020.

Plan of Correction: The facility responded: For the future, I plan on meeting the requirements for all fire drills, lock down drill and shelter in place drill for each month. Alternating to proceed in drills during the morning or afternoon times each month. Recording all months of each drill required for licensing on our sheet that is placed on the licensing board.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of four staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment.

Evidence:
1. The record for staff #1 (date of hire 8/17/20) did not contain documentation of a completed search of the central registry finding.
2. The record for staff #2 (date of hire 8/13/20) did not contain documentation of a completed search of the central registry finding.
3. Staff #4 (Program Director) reviewed the records for the staff #1 and staff #2, and confirmed that the search of the central registry finding has not been received for either staff.

Plan of Correction: The facility responded: I have created a staff information sheet where I will record all correct staff information including date of employment, job title, date of sworn disclosure, date of current central registry, criminal history and update TB test. I will attached that sheet with the email. Meeting all requirements for turning in and receiving results of all background checks and making sure staff can?t start until I receive one of the documents.

Description: Based on a review of four staff records, it was determined that the facility did not ensure that an employee is allowed to begin employment without a completed national criminal history record check (finger printing).

Evidence:
1. The record for staff #2 (date of hire 8/13/20) contained a completed national criminal history record check (finger printing) that was dated 8/20/20.
2. Staff #4 (Program Director) confirmed that the results of the finger print background check (finger printing) were not received prior to staff #2 beginning employment.

Plan of Correction: The facility responded: I have created a staff information sheet where I will record all correct staff information including date of employment, job title, date of sworn disclosure, date of current central registry, criminal history and update TB test. I will attached that sheet with the email. Meeting all requirements for turning in and receiving results of all background checks and making sure staff can?t start until I receive one of the documents.
