This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 9/23/2020 and concluded on 9/24/2020. The director was contacted by telephone to initiate the inspection. There were 22 children present and 4 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Friday 10/2/2020. Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review and staff interview, the center did not request the results of an out of state central registry search within 30 days of employment for all employees who have resided outside the state of Virginia within the last 5 years.
Evidence: Staff #1 (Date of Hire: 6/8/2020) has resided outside the state of Virginia within the last 5 years as indicated on the Sworn Disclosure Statement signed on 6/8/2020. The out of state central registry search request form on file is dated 9/24/2020.Plan of Correction: Form mailed on 9/24/20 to arrive by 9/25/20 12:00pm to out of state CPS.
