Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on September 18, 2020 and concluded on September 21, 2020. The provider was contacted by telephone to initiate the inspection. There were 27 children present and 5 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed three children?s records and three staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Description: Based on record review and interview, the facility did not obtain a sworn statement prior to hire for one of three employees.

Evidence:
The record for staff #2 (DOH 03/12/2020) had documentation of a sworn statement on 03/17/2020.
