63.2(17) License & Registration Procedures

Comments:
The inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on September 18, 2020 and concluded on September 25, 2020. The director was contacted by telephone to initiate the inspection. There were 29 children present with seven staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed three children records three staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's files, the center failed to ensure that each child enrolled had all required information in their record.

Evidence:
1. Child #1's file did not contain an address for one emergency contact. Date of enrollment for Child #1 was 09/10/2020.
2. Child #2's file did not contain a complete address for one emergency contact. Date of enrollment for Child #2 was 09/10/2020.
3. Staff verified that the records did not contain all required information for emergency contacts.

Plan of Correction: Administration will speak with the parents at pick up and obtain the emergency address for the emergency contacts.

Standard #: 63.2(17)-1720.1-A
Description: Based on a review of staff records, the center failed to obtain a required updated central registry background check every five years.

Evidence:
1. Staff #3's central registry check on file was dated 08/07/2015. An updated central registry check was due by 08/07/2020.
2. Staff verified that the central registry check dated 08/07/2015 was the most current central registry check obtained, and an update had not been completed.

Plan of Correction: Administration will have Staff #3 complete the required paperwork and send out the background check immediately. Administration will provide proof to the Licensing Inspector within 10 business days that the central registry check has been sent out or completed.
