This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 09/21/2020 and concluded on 09/29/2020. The provider was contacted by telephone to initiate the inspection. There were no children present during the inspection as the program is currently closed due to COVID-19; the program has a reopening date of 10/12/2020 when public school resumes. There was 1 staff member present during the inspection. The inspector reviewed 2 children's records, 2 staff records and 5 board officer/agent records during the inspection to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-191-40-D-1-A Description: Based on record review and discussion with the staff, the facility failed to obtain a sworn statement/affirmation and search of the central registry for new persons designated as applicants or agent who is or will be involved in the day-to-day operations of the facility before the end of 30 days after the change.
Evidence: There was not a sworn statement/affirmation or a search of the central registry available for review for Agent #2; Agent #2 took office in 02/2020. Staff #3 confirmed that those documents had not been obtained.Plan of Correction: The agents were recontacted and requested to complete the paperwork immediately. In the future Staff #3 will attend the first Board Meeting and obtain all necessary paperwork at that time.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on record review and discussion with the staff, the facility failed to ensure that all applicants/agents submit to fingerprinting as required by the Code of Virginia.
Evidence: There was no fingerprint eligibility letter to verify that Agent #4 had submitted to fingerprinting; Agent #4 took office in February 2020. Staff #3 confirmed that the fingerprints had not been submitted to Fieldprint.Plan of Correction: The agents were recontacted and requested to complete the paperwork immediately. In the future Staff #3 will attend the first Board Meeting and obtain all necessary paperwork at that time.
