Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on September 16, 2020 and concluded on September 24, 2020. The Program/Center Director was contacted by telephone to initiate the inspection. There were 35 children present and 5 staff on September 16, 2020. The inspector emailed the Program/Center Director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:

Standard #:	22VAC40-185-130-A
Description:	Based on documentation review and staff interviews, the licensee did not ensure to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: The record for child #2 did not include a copy of immunizations and first date of attendance was 6-15-2020.
Plan of Correction:	Will make sure to have a copy of immunizations for each child.
Standard #:	22VAC40-185-140-A
Description:	Based on documentation review and staff interviews, the licensee did not ensure that each child shall have a copy of a physical examination by or under the direction of a physician in their record within one month after attendance. Evidence: The record for child #2 did not include a copy of physical and first date of attendance was 6-15-2020.
Plan of Correction:	Will make sure to have a copy of a physical for each child .
Standard #:	22VAC40-185-240-A
Description:	Based on review of 3 staff records and staff interviews, the licensee did not ensure all staff had orientation training completed by the end of their first day of assuming job responsibilities. Evidence: The records for staff #1, staff #2 and staff #3 did not include documentation of orientation training completed by the end of the first day of assuming job responsibilities. The center director confirmed they were working during the inspection.
Plan of Correction:	Will ensure that each staff has proper documentation of orientation.
Standard #:	22VAC40-185-550-D
Description:	Base on documentation review and staff interviews, the licensee did not ensure to monthly practice evacuation drill and a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios. Evidence: There center director confirmed there was not an evacuation drill practiced in August 2020.
Plan of Correction:	Will ensure that safety drills are held and documented each month.
