Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency heath pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 09/21/2020 and concluded on 10/08/2020 in response to a self-reported incident concerning the staff-to-child ratio in one classroom. Based on the information gathered, which included staff interviews and a review of written attendance records, this report is found to be valid. The violation is documented on the violation notice issued to the facility.

Description:	Based on staff interview and record review, the center failed to ensure that the following ratios of staff to children are required whenever children are in care: *For children from birth to the age of 16 months: one staff member for every four children. Evidence: 1. Administrative staff 1 reported to the inspector that classroom 8 exceeded the required staff-to-child ratio on 09/21/2020 from approximately 9:06 am - 11:19 am. a. There were a total of nine children, ages 15 months - 24 months, with 2 staff at 9:06 am. A 10th child arrived at 9:16 am. b. Three staff were needed as one child in the group, child 1, was 15 months of age which requires a staff-to-child ratio of 1:4. 2. Administrative staff 1 stated that child 1 was moved to the infant classroom at 11:19 am when classroom staff noted child 1's actual birthdate.
Plan of Correction:	*Created a Licensing and Compliance Officer position. An individual was promoted to that position. 09/21/2020 *Employee for this violation is no longer employed. 09/30/2020 *Redesigned classroom ratios to emphasize birthdates and ratios based upon age of child. 09/22/2020 * Created and implementing two signature requirement on new student file checklist form to cross check accuracy. 10/12/2020 * Held a staff meeting, Monday, October 12, 2020 to review licensing protocol and emphasize every staff person is responsible for compliance.
