Technical Assistance:
Discussed new requirements for out-of-state background checks, requirements for physical examinations for children, and the waivers on child care regulations during the COVID pandemic.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 10/21/2020 and concluded on 10/28/2020. The in-charge person was contacted by telephone to initiate the inspection. There were 9 children present and 2 staff. The inspector emailed the in-charge person a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-140-B
Description: Based on record review, it was determined that a physical examination done prior to attendance was outside of the time period allowed for children 2 to 5 years of age.
Evidence:
1) Child #2 (dob 07/21/2016) first date of attendance is 09/09/2020. The physical examination available for review in Child #2's record was dated 10/24/2019, which is more than the12 months period allowed for children 2 to 5 years of age.

Plan of Correction: I will let the teachers know so they can request the parents for a current physical examination for Child #2.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain work phone numbers for both custodial parents. The record for Child #2 did not contain a work phone number for one custodial parent.

Plan of Correction: I will send the information to the teachers to have parents provide the information.

Standard #: 22VAC40-185-90--A
Description: Based on record review, it was determined that written agreements between the parent and the center were not in each child's record.
Evidence:
1) The record for Child #2 did not contain documentation to show that require written agreements between the parent and the center were signed by the first day of the child's attendance.

Plan of Correction: I will send this information to the teaches to have the parents sign the required forms.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview, it was determined that the center did not ensure that Central Registry search results are obtained every five years for employees.
Evidence:
1) The most recent Central Registry search request results available in the record for Staff #2 were dated 10/08/2015. A new Central Registry search request was due on 10/09/2020.

Plan of Correction: I will ask Staff #2 to come to the office to sign the form needed to request the background check.
