This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring (focus) inspection was initiated on October 26, 2020 and concluded on October 29, 2020. The provider was contacted by telephone to initiate the remote inspection. The inspector emailed the provider a list of items required to complete the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-150-B Description: Based on record review, in one of two children's records reviewed, the licensee did not ensure that each medical report shall be signed by a physician, his designee, or an official of a local health department.
Evidence: The licensee confirmed that the immunization report for Child #2 had not been signed by a physician, his designee, or an official of a local health department.Plan of Correction: We will get either the immunization signed or stamped.
Standard #: 22VAC40-185-60-A Description: Based on record review, in one of two children's records reviewed, the licensee did not ensrue each enrolled child's record contained all required information.
Evidence: The licensee confirmed that in the record for Child #2 the work phone numbers for both parents had not been documented.Plan of Correction: We will ensure that the work phone number is indicated and if it is the same as the cell phone it will be repeated in that data field or N/A will be inputted.
