Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 10/16/2020 and concluded on 10/22/2020. The provider was contacted by telephone to initiate the inspection. There were 16 children present with 5 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-500-F
Description: Based on interview and observation, the vendor failed to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.

Evidence:
1. There was no allergy list available for child 1 who has a diagnosed food allergy.
a. Administrative staff stated that an allergy list was not available.

Plan of Correction: An allergy list will be written and placed in an area that is confidential but accessible to all staff.

Standard #: 22VAC40-665-580-B
Description: Based on record review and interview, the vendor failed to ensure that within the first 90 days of employment all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff.

Evidence:
1. Staff 1 has not completed Virginia Preservice Training according to written training records.
a. Staff 1 has a hire date of August 2019
2. Administrative staff verified that staff 1 had not completed Virginia Preservice Training.

Plan of Correction: Staff 1 is working on this training and should have completed within 30 days.

Standard #: 22VAC40-665-580-C
Description: Based on record review and interview, the vendor failed to ensure that staff complete the annual Virginia Health and Safety Update Training for Child Care Staff.

Evidence:
1. Staff 2 and staff 3 have not completed the annual Virginia Health and Safety updated training for child care staff according to a review of training documentation.
a. This training was required to be completed by 03/01/2020.
2. Administrative staff verified that staff 2 and staff 3 had not completed this training.

Plan of Correction: Staff 2 is working on this training and should have it completed within 30 days.
Staff 3 is on a leave of absence and will complete if she returns to work.

Standard #: 22VAC40-665-580-E-1
Description: Based on record review and interview, the vendor failed to ensure that staff obtain current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 90 days of employment.

Evidence:
1. Staff 1 lacks current CPR certification. Staff 1 has a hire date of August 2019.
2. Administrative staff confirmed that staff 1 lacks current CPR certification.

Plan of Correction: Staff 1 will obtain CPR certification as soon as the next class is available.

Standard #: 22VAC40-665-580-E-2
Description: Based on record review and interview, the vendor failed to ensure that staff obtain current certification in first aid appropriate to the age of children in care within 90 days of employment.

Evidence:
1. Staff 1 lacks current first aid certification. Staff 1 has a hire date of August 2019.
2. Administrative staff confirmed that staff 1 lacks current first certification.

Plan of Correction: Staff 1 will obtain first aid certification as soon as the next class is available.
