Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 11/03/2020 and concluded on 11/06/2020. The director was contacted by telephone to initiate the inspection. There were 29 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. The information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Standard #:	22VAC40-185-70-A
Description:	Based on review of two staff files, the facility failed to have all required documents in each staff file. Evidence: Staff A and Staff B did not have documentation of two references before employment as required. Staff A did not have documentation of orientation by the end of the first day of employment.
Plan of Correction:	Two references will be obtained for each staff prior to hire. Orientation will be documented for each staff by the end of the first day of employment.
Standard #:	22VAC40-191-60-B
Description:	Based on review of two staff files, the facility failed to have a signed sworn disclosure statement prior to the first day of employment. Evidence: Staff B (date of hire 12/2019) did not have documentation of a sworn disclosure statement.
Plan of Correction:	All staff will complete a sworn disclosure statement prior to the first day of employment.
Standard #:	22VAC40-191-60-C-2
Description:	Based on review of two staff files, the facility failed to have documentation of a search of the central registry by the 30th day of employment. Evidence: Staff B (date of hire 12/2019) did not have documentation of a search of the central registry (CPS check).
Plan of Correction:	CPS checks will be mailed within the first 7 days of hire and will be followed up on if not received by the 30th day of employment.
Standard #:	63.2(17)-1720.1-A
Description:	Based on review of three staff files, the facility failed to have documentation of a repeat search of the central registry every 5 years for each staff. Evidence: the search of the central registry (CPS check) for Staff C expired 4/8/2020 and has not been repeated.
Plan of Correction:	The CPS check will be sent for Staff C.
Standard #:	63.2(17)-1720.1-B-2
Description:	Based on review of two staff files, the facility failed to obtain fingerprint results for each staff prior to hire. Evidence: Staff B (date of hire 12/2019) did not have documentation of fingerprint results.
Plan of Correction:	Fingerprints will be obtained for each staff prior to hire.
