Standard #: 63.2(17)-1721.1-B-4 Description: Based on staff interview and record review, the center did not obtain a copy of the results of a criminal history record information check from any state in which the individual has resided in the preceding five years, before the first day of employment.
Evidence:
1. Staff # 1's records indicate that her first date of employment was 10/5/20, and that she had resided outside of Virginia within the last 5 years.
2. When asked by the licensing inspector, Staff # 1 confirmed that her first date of employment was 10/5/20.
3. A review of Staff # 1's out of state criminal history record check showed that it was requested and received on 11/12/20.Plan of Correction: Out of state background checks will be completed on time.
