Technical Assistance:
Discussed with the director procedures for drills as well as documentation, and provided additional technical assistance for children's files and linens.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A focused monitoring inspection was initiated on 11/12/20 and concluded on 11/16/20. The director was contacted by telephone to initiate the inspection. There were four children present being supervised by one staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on documentation review, the center did not obtain all of the required documentation for each child enrolled. Evidence: The record for Child A did not contain documentation of the work phone number or place of employment for either parent.

Plan of Correction: We will obtain the information from the parents.

Standard #: 22VAC40-185-450-A
Description: Based on staff interview, the center did not ensure that rest mats used by children had linens consisting of a top cover and a bottom cover or a one-piece covering open on three edges. Evidence: Staff stated the children did not have any linens that they used with their rest mats.

Plan of Correction: We now provide top and bottom covers for the children for rest time that we launder nightly.

Standard #: 22VAC40-185-550-D
Description: Based on review of documentation and staff interview, the center did not practice monthly evacuation drills and a minimum of two shelter-in-place drills per year. Evidence: 1. Staff stated that shelter-in-place drills are practiced once per year. 2. Documentation submitted for the monthly evacuation drills only included children practicing the drill on November 12, 2020. The rest of the documentation of drills submitted showed no children practiced the drill.

Plan of Correction: A shelter-in-place drill was conducted on 11/16/20 and documentation was submitted to the Licensing Inspector. We will make sure drills are conducted and documented as required.
