Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint investigation was initiated on November 20, 2020 and concluded on November 23, 2020. A complaint was received by the department regarding allegations in the areas of health records and allergies. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.

The evidence gathered during the investigation did not support the allegation of non-compliance with standards or laws. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice that was issued to the facility.

Description: Based on review of children's records, the center failed to ensure that all required information was in a child's file.

Evidence:
1. The contact information and home address for Child #1's mother was not listed on the registration form.
2. The home address for Child #1's father was not listed on the registration form.
3. There was no place on the registration form to indicate who had legal custody of the child and their contact information.
4. There was not a contact number listed for the physician.
5. The two emergency contacts listed had the same address and same phone number.
6. Staff verified that this was the most current registration form that they had on file for all the children and the Child #1's file was missing the required information.

Plan of Correction: Administration will edit the current registration form to include all required information and have all parents complete a new form. The physician's phone number has been added, and administration will contact the parent and request either another emergency contact or another phone number to contact the individuals listed.
