Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on November 19, 2020 and concluded on November 20, 2020. The director was contacted by telephone to initiate the inspection. There were 81 children present and 18 staff. The Inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records submitted by the facility to ensure documentation was complete.


Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22VAC40-185-150-B
Description: Based on a review of records on November 20, 2020, the center did not ensure that immunizations received shall be signed by a physician, his designee, or an official of the local health department.
Evidence: The record of Child #4 contained documentation of immunizations that were not signed by a physician, his designee, or an official of the local health department.

Plan of Correction: Starling always ensures that immunization records are received and complete each page is signed by a physician.

Standard #: 22VAC40-185-70-A
Description: Based on a review of records on November 20, 2020, the center did not ensure that each staff record contains all the required information.
Evidence: The records of Staff #3 and Staff #4 contained documentation of telephone references that were not signed by the person taking the reference.

Plan of Correction: Starling always checks references for each new employee noting the date and signature of person obtaining documentation.

Standard #: 22VAC40-191-60-B
Description: Based on a review of records on November 20, 2020, the center did not ensure to obtain a Sworn Statement from each staff prior to employment.
Evidence: The record of Staff #5 (DOH 10/31/20) contained documentation of a Sworn Statement dated 11/2/20.

Plan of Correction: Starling always does a thorough background check including a sworn statement prior to hiring staff.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of records on November 20, 2020, the center did not ensure to obtain a fingerprint based national criminal record check prior to the first day of employment for each staff.
Evidence: The record of Staff #4 contained documentation of fingerprints dated 8/27/20. Staff #4 completed orientation on 8/14/20.

Plan of Correction: Starling always ensures that a new staff member obtains a fingerprint prior to the first day of employment.
