Technical Assistance:

Due to the background check related violations cited in today?s inspection, the provider is required to follow-up with the appropriate department regarding the status of any missing and/or incomplete background checks. Documentation of the completed follow-up must be sent to the Licensing Inspector within 10 business days, and a copy of all missing/incomplete background check results must be sent to the Licensing Inspector upon arrival.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A virtual renewal inspection was initiated on 11/30/2020 and concluded on 12/2/2020. The director was contacted by telephone to initiate the inspection. There were 25 children present and 14 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #:	22VAC40-185-550-D
Description:	Based on record review, the center did not implement a monthly practice of evacuation drills. Evidence: There was no documentation of a completed fire drill for November 2020.
Plan of Correction:	We will do one today.
Standard #:	22VAC40-191-40-D-1-A
Description:	Based on a review of board member records, the child day center did not obtain central registry results for board members within 30 days of taking office. Evidence: Board member C and Board member D's records did not contain documentation of central registry results.

Description:	Based on a review of staff records, the child day center did not obtain a copy of information from the central registry for one staff member. Evidence: Staff A's (date of hire: 9/28/2020) record indicated that the employee has resided in the District of Columbia within the past five years. The employee's record did not contain documentation of a central registry search for the District of Columbia.

Description:	Based on a review of staff and board member records, the child day center did not obtain a copy of criminal history and sex offender registry results from any state in which the applicant has resided in the preceding five years. Evidence: 1. Staff A's (date of hire: 9/28/2020) record indicated that the staff member has resided in the District of Columbia within the past five years. The employee's record did not contain documentation of a sex offender search and criminal history search for the District of Columbia. 2. Board member A and Board member C's records indicated that the individuals have resided in Maryland within the past five years. Neither board member records contained documentation of of Maryland sex offender registry search results.
