This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 11/25/2020 and concluded on 12/4/2020. The director was contacted by telephone to initiate the inspection. There were 30 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4pm on Friday 12/11/2020. Standard #: 22VAC40-185-140-A Description: Based on record review, 1 of 3 children's records did not contain documentation of physical examination within one month after the child's first day of attendance.
Evidence: Child #3 (First day of attendance: 12/9/2019) did not have a physical examination on file.Plan of Correction: I have requested a copy of the physical from the parents.
