Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on December 3, 2020. The program director was contacted by telephone to initiate the virtual, remote inspection. There were 27 children present with six staff. The inspector reviewed a list of items required to complete the inspection. The inspector reviewed three children?s and staff records during the virtual, remote inspection to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on observation the center failed to ensure that children under 10 years of age always are within actual sight and sound supervision of staff.
Evidence:
1 - Staff 2 was observed sitting between two toddler classrooms, in a door way. There were four children in each room between the ages of 18months and two years old. The rooms are divided by a half wall. In one room a child was awake and moving around the room. While staff 2 was attending to the child in this room, the children in the other were not under sight supervision.
2 - Staff 3 was observed sitting between two classrooms, in the door way. The rooms are divided by a half wall. One classroom contained seven, two year old children and the other classroom had two, four year old children

Plan of Correction: Staff will be retrained in the proper methods for supervising children while maintaining ratios. Staff will no longer be allowed to sit between to classrooms at nap time.

Standard #: 22VAC40-185-500-A
Description: The center failed to demonstrate that staff shall wash their hands with soap and running water before and after helping a child use the toilet or a diaper change.
Evidence:
1 - Staff 1 did not wash her hands after changing the diaper of child 1 before assisting child 2 with diaper changing.
a. Staff 1 was observed assisting child 1 with pulling her pants up after changing the child's diaper. However, once she finished with child 1 she immediately picked up child 2 to lift him onto the changing table.
2 - Staff 1 stated that she did not wash her hands between assisting child 1 and 2 with diaper changes.

Plan of Correction: Staff will be retrained and reminded in proper hand washing procedures as related to diaper changing.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interview the center failed to demonstrate that there will be sight and sound supervision for all children when a child is being diapered. Additionally, the center failed to ensure that staff will ensure the immediate safety of a child during diapering.
Evidence:
1 - Upon entering the two year old room there were three children sitting; two on the floor and one on a table. There was not a staff in room.
2 - Staff 1 was in the bathroom with child 1 and 2. While the bathroom is located within the classroom, staff 1 was not able to maintain sight and sound supervision of the three children in the classroom while she was completing diaper changing in the bathroom.
3 - Child 2 was on the changing table when staff one turned away from the changing table and bent down assist child 1 with pulling up her pants. During this time child 2 was left unsupervised while laying on the changing table which is attached to the wall approximately four feet above the floor.
3 - Staff 1 and 4 confirmed that staff 1 could not see the children left in the classroom while in the bathroom changing diapers.
4 - Staff 1 and 4 confirmed that child 2 was left on the diaper changing table while staff 1 assisted child 1 who was standing on the floor.

Plan of Correction: Staff will ensure that all children are within sight and sound supervision during all diaper changing.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review the center failed to ensure that out of state background checks are obtained prior to employment.
Evidence:
1 - The record for staff 5 does not contain any documentation that an out of state background check was requested.
a. The sworn statement or affirmation states that staff did live outside of Virginia within the past five years.
b. Staff 5 was present and observed caring for children on December 2, and 3, 2020.

Plan of Correction: Within ten days of receipt documentation that the background check has been requested and or results received.
