Evidence: The record for Staff #2 did not contain documentation that two or more references as the character and reputation as well as competency were checked before employment.Plan of Correction: References will be obtained.
Standard #: 22VAC40-185-550-D Description: Based on document review and discussion with staff, the facility failed to implement a monthly evacuation drill.
Evidence: Monthly evacuation drills were not implement in October 2020 or November 2020.Plan of Correction: Drills will be done monthly. A reminder will be sent to all directors.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review and discussion with the staff, the facility failed to obtain a search of the central registry and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any found complaint of child abuse or neglect against them.
Evidence: The record for Staff #2 had documentation of residing outside of the state of Virginia within the preceding five years. There was no documentation of any child abuse and neglect registry or equivalent registry maintained by the other state listed. The documented date of hire for Staff #2 was 08/17/2020.Plan of Correction: The out of state check will be submitted.
