The licensee has been informed that they must provide missing background check documentation within 10 business days of the inspection. Failure to do so could result in a potential enforcement action against the licensee.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 11/23/2020 and concluded on 11/23/2020. The director was contacted by telephone to initiate the inspection. There were 65 children present and 20 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Evidence:
Child#1's record is missing the parents' work phone numbers.Plan of Correction: We have asked the family to provide the numbers.
Standard #: 22VAC40-185-260-A Description: Based on record review and interview, the annual fire inspection is expired.
Evidence:
The last fire inspection was dated 09/01/2019, the center's fire inspection expired on 09/01/2020.Plan of Correction: We have made several phone calls to get in contact with the fire department. We are waiting for a phone call from them to get the facility inspected.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on staff record review, there is no documentation that Child Abuse and Neglect results were obtained for employees that have resided outside the state of Virginia within the last 5 years.
Evidence:
Staff#3 (DOH: 08/30/2019) has resided outside the state of Virginia within the last 5 years, their records did not include documentation to verify that out-of-state child abuse and neglect results were obtained.Plan of Correction: We have given the form to the employee to complete, we will send it out tomorrow.
