Technical Assistance:
Approved Subsidy Vendors are required to notify the department of the occurrence of an Injury Requiring Outside Medical Treatment (IROMT) as soon as practicable, but not to exceed two (2) business days of the occurrence. Serious injury means an injury of a child under the care of the center that requires outside medical treatment. Reports can be submitted online at: https//www.dss.virginia.gov/facility/injury_report_subsidy.cgi

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy complaint inspection was initiated on December 14, 2020, December 15, 2020 and concluded on December 18, 2020. A subsidy complaint was received by the department regarding allegations in the areas of supervision. The provider was contacted by telephone to conduct the investigation. The inspector emailed the provider a list of documentation required to complete the investigation.

The evidence gathered during the investigation supported the allegation of non-compliance with subsidy regulations, and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Violations:
Standard #: 22VAC40-665-550-2
Description: Based on interviews and documentation review, the center did not notify the department within two business days of a serious injury to a child - while under the center's supervision - that required outside medical attention.

The center did not report the injury to the licensing inspector or complete an Injury Requiring Outside Medical Treatment (IROMT) online.
