Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was initiated on 12/1/20 and concluded on 1/19/21. A complaint was received by the department on 11/23/20 regarding allegations in the areas of supervision. The director/provider or in-charge person was contacted by telephone to conduct the investigation on 12/1/20. The inspector emailed the director/provider a list of documentation required to complete the investigation. The evidence gathered during the investigation did not support the allegation of non-compliance with standards or law. However, violations were identified during the course of the investigation, and can be found on the violation notice.

Description: Based on a review of one child's record, it was determined that the facility did not ensure that each child in attendance had a completed physical within one month of attendance.

Evidence:
1. The record for child #1 did not contain a physical examination.
2. Staff #2 (Assistant Director) confirmed that the record for child #1 did not contain a physical examination, and had been enrolled for more than 30 days.

Plan of Correction: The child is no longer enrolled at Toddler Station as of November 19, 2020. Future action will be that the director/assistant director will have a form in place for all newly enrolled children to make sure all the proper paperwork is in the child's file in the proper time frame required.

Description: Based on a review of one staff record, it was determined that the facility did not ensure that at least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence:
1. The record for staff #1, contained documentation of TB screening that was dated 8/24/18.
2. Staff #2 (Assistant Director), reviewed the record for staff #1 and confirmed that an updated TB screening had not been received.

Plan of Correction: The facility responded: The director/assistant director will go over the checklist monthly to ensure all staff have all the required (paperwork, training's, and test) to maintain employment.
