8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Comments:
An unannounced subsidy health and safety inspection (SHSI) was initiated and concluded on June 27, 2022. The licensing inspector was onsite from approximately 11:30am until 1:10pm. There were two children in care one caregiver present. A monitoring inspection was also conducted on this date.

The information gathered during the inspection determined non-compliances with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-790-190-A
Description: Based on interview and documentation review, the vendor did not maintain, and keep at the family day home, written or electronic information for each enrolled child.

Evidence:
Two children were in care during the June 27, 2022 renewal inspection. The licensing inspector asked to review the record for Child #1. Vendor #1 stated Child #1 recently started and she did not have a record for Child #1.

Plan of Correction: The child just started today. I will make sure the parents complete the required paperwork.

Standard #: 8VAC20-790-190-B-12
Description: Based on record review, the vendor did not obtain a written care plan for one child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:
The record for Child #2 indicated the child has a food allergy. The record did not contain a written care plan completed by a physician.

Plan of Correction: I will give parent's an allergy action plan form for the doctors to complete.

Standard #: 8VAC20-790-270-A
Description: Based on observation, the vendor did not ensure all cleaning agents were stored in areas inaccessible to children.

Evidence:
Two containers for cleaning chemicals were observed sitting on surface near the kitchen. The area was accessible to the two children in care.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-790-360-G
Description: Based on observation and interview, the vendor did not ensure cots, rest mats, or beds were provided for children 12 months of age or older.

Evidence:
When the inspector arrived at the home, Child #1 (14 months of age) was observed sleeping on the couch. Vendor #1 stated she needed to get a mat out for Child #1. Child #1 was moved to a mat during the inspection.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-790-400-H
Description: Based on observation and interview, the vendor did not ensure medications for caregivers and household members, were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. During the renewal inspection, a bottle of over-the-counter cold medication was sitting on table by the couch where children were located.
2. The vendor stated the medication belonged to her.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-790-470-C
Description: Based on interview, the vendor did not ensure all dogs or cats were vaccinated for rabies and be treated for fleas, ticks, or worms as needed.

Evidence:
Two cats were present in the home, during the inspection. The vendor stated the two cats are house pets. When licensing inspector asked to review the rabies shot record, the vendor stated the cats had not received rabies shots.

Plan of Correction: I will contact local vet and rabies clinics and schedule an appointment for the cats.
