Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on January 8, 2021 and concluded on January 8, 2021. The director was contacted by telephone to initiate the inspection. There were 22 children present and 7 staff. The Inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #:	22VAC40-185-550-D
Description:	Based on a review of records and interview on January 8, 2020, the center did not ensure to implement a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios. Evidence: 1. There was documentation of one shelter-in-place drill dated 10/6/20. 2. Administration stated there was no Spring drill due to covid-19 and that she thought the drills had to be based on the school year.
Plan of Correction:	Dates for 2 shelter in place drills will be set at the beginning of each calendar year. This requirement will be added to the Operations Manual.
Standard #:	22VAC40-191-40-D-1-A
Description:	Based on a review of records and interview on January 8, 2021, the center did not ensure to obtain a Sworn Statement and a central registry check from each agent of the organization before the end of the 30 days after the change of agent. Evidence: The record of Agent #1 did not contain documentation of a sworn statement or central registry results. 2. Administration confirmed the sworn statement and central registry check for Agent #1 were not completed.
Plan of Correction:	The current trustees and Senior Pastor have been instructed that when a trustee informs New Highland Baptist that they are leaving the position, the director of The Day School will be told of the new appointment so appropriate background checks and references can commence.
Standard #:	63.2(17)-1721.1-B-2
Description:	Based on a review of records and interview on January 8, 2020, the center did not ensure to obtain a fingerprint based national criminal record check within 30 days of appointment for each agent. Evidence: 1. The record of Agent #1 (date of appointment 5/19/20) did not contain documentation of fingerprints. 2. Administration confirmed the fingerprints were not obtained.
Plan of Correction:	The current trustees and Senior Pastor have been instructed that when a trustee informs New Highland Baptist that they are leaving the position, the director of The Day School will be told of the new appointment so appropriate background checks and references can commence.
