This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 2/10/2021 and concluded on 2/12/2021. The person in charge was contacted by telephone to initiate the inspection. There were 51 children present with 12 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with subsidy regulations and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-665-540-A Description: Based on record review and interview, the center failed to ensure staff obtained a statement signed by a physician, physician's designee, or an official of the local health department determined to be free of communicable tuberculosis (TB) within the last 30 calendar days of the date of employment and submitted prior at the time of employment.
Evidence:
1. The record for staff 1 documents the start date as 11/24/20. The center has no statement that staff 1 is free of TB.
2. The director verified they do not have a statement staff 1 is free of TB. Staff 1 provided documentation of obtaining TB shot but did not return to obtain the TB reading.Plan of Correction: Staff 1 will be required to provide documentation of TB results. In the future the center will ensure all staff provide documentation of TB results prior to employment & contact with children.
